Does a guarantor have to sign Tenancy Agreement?

Yes, they do. You should have a clause in the agreement regarding a guarantor and there should be a space for them to sign it and ideally get somebody to witness it.

The guarantor should have the opportunity to read and raise questions about the tenancy agreement prior to signing the guarantee. As long as you have good evidence that this was offered/done, its not essential that they sign the TA, although signing it is itself good evidence.
