HMO rules - how to interpret them

Hi, I am trying to check HMO rules which I know can vary by local council. My scenario is I have a a 2 bed, 2 bath purpose built flat and have three adult tenants interested. Two are a couple (I think the term is cohabiting!) and one of there cousins. My query is does this constitute one or two households under HMO rules? and also there seems to be different thresholds for a HMO - 3 and 4 people.
