Moving to a periodic tenancy after 12 month AST but increasing rent

The tenant has agreed to the monthly rent increase. Do I need to complete Form 4 from Govt website or should I just send a letter confirming the new amount and payment start date?

The latter. No need for form 4 if they’ve agreed and start paying.
